Mother Cabrini Health Foundation, Inc. (“Foundation,” “we,” “our” or “us”) is committed to safeguarding the privacy of the confidential and sensitive information in our care. This Privacy Policy (“Privacy Policy”) explains how we collect and handle information that you provide to us through our Grant Portal (located at https://cabrinihealth.smartsimple.us) (collectively, the “Site”) which is owned and operated by SmartSimple Software, Inc. (“SmartSimple”) pursuant to a contractual arrangement between SmartSimple and the Foundation. This Privacy Policy applies to the Site and all services provided by the Foundation through the Site.
Please read this Privacy Policy carefully because when you submit information to or through the Site, you are consenting to the collection, processing and retention of your information as described in this Privacy Policy. Capitalized terms used but not defined herein have the meanings assigned to them in SmartSimple’s Website Terms and Conditions of Use.
If you do not agree with any part of this Privacy Policy, please stop using the Site immediately.
INFORMATION WE COLLECT:
The Site is used to receive, evaluate and process your application for funding from the Foundation (hereinafter referred to as a “Funding Application”).
The Site collects the information that we need to evaluate and review your Funding Application. The information that we collect includes contact and identity information, as well as financial and operational information about the organization requesting funding (all such information is referred to as “Confidential Information”). You can choose whether or not to disclose your Confidential Information; however, if you do not provide all of the information that we have deemed necessary, you will not be able to take full advantage of the Site and we will not be able to evaluate or review your Funding Application.
By submitting Confidential Information and using the Site, you consent to the collection, use, processing, reproduction, hosting, transmission, and disclosure of any such submissions, as we deem necessary to accomplish the purpose described in this Privacy Policy.
By using this Site you are also consenting to the use of cookies, tracking pixels or other technologies which allow servers to recall previous requests or registration and/or IP addresses. If you have provided us with your Confidential Information, we may associate this Confidential Information with information we have collected through cookies, tracking pixels or other technologies. You can set your browser to notify you before you receive a cookie, giving you the chance to decide whether to accept it. You can also set your browser to turn off cookies. The Site may automatically collect your IP address or other unique identifier (“Device Identifier”) from the computer, mobile device or other device (collectively, “Device”) you use to access the Site. A Device Identifier is automatically assigned to your Device when you access a website or its servers, and our computers identify your Device by its Device Identifier. We may use a Device Identifier to, among other things, administer the Site, help diagnose problems with our servers or analyze trends.

HOW WE SHARE YOUR INFORMATION:
We do not sell, trade, or rent your Confidential Information to others. However, we may share your Confidential Information under the following circumstances:
Service Providers. We may share your Confidential Information with SmartSimple or other third parties that perform functions on our behalf (collectively, “Service Providers”) to evaluate or review your Funding Application, respond to your inquiries or to make various programs, services or features of the Site available to you. We may also share your Confidential Information with Service Providers that help us administer the grant application process and related transactions. Service Providers may also help us to host or operate the Site, analyze data, provide customer service, administer surveys and questionnaires or assist with marketing and raising awareness about the Foundation and its programs, services or mission. We take commercially reasonable efforts to confirm that these Service Providers are only using your Confidential Information to perform specified functions on our behalf.
Business Transfers. We may share your information internally, including your Confidential Information, primarily for business and operational purposes. We may share or transfer your information, including your Confidential Information in connection with a merger, sale, acquisition, divestiture, restructuring, reorganization, dissolution, bankruptcy or other change of ownership or control by the Foundation or any affiliated entity (in each case, whether in whole or in part).
Legal Disclosure. We may transfer and disclose information, including your Confidential Information, to third parties to comply with a legal obligation, when we believe in good faith that the law requires it, at the request of governmental authorities conducting an investigation, to verify or enforce our Terms of Use, Privacy Policy or other applicable policies, to respond to an emergency or otherwise to protect the rights, property, safety, or security of the Foundation, third parties, visitors to the Site or the public.
YOUR PRIVACY CHOICES AND ACCESS:
If you wish to verify, correct, or update any of your Confidential Information collected through the Site, you may contact us. In accordance with our routine record keeping, we may delete certain records that contain Confidential Information that you have submitted through the Site. We are under no obligation to store such Confidential Information indefinitely and disclaim any liability arising out of, or related to, the destruction of such Confidential Information. In addition, you should be aware that it is not always possible to completely remove or delete all of your information from our databases without some residual data because of backups or other technology-related reasons.
ACCESSING OUR SITE:
The Site is governed by and operated in, and in accordance with the laws of the United States. We make no representation that the Site is governed by or operated in accordance with the laws of any other nation. By using the Site, or providing us with any information, you (a) acknowledge that the Site is subject to the laws of the United States, (b) consent to the collection, processing, maintenance and transfer of such information in and to the United States, and (c) waive any claims that may arise under the laws of any other jurisdiction.
CALIFORNIA PRIVACY RIGHTS:
California Civil Code Section 1798.83 permits California residents to request certain information regarding our disclosure of Confidential Information to third-parties for their direct marketing purposes during the preceding calendar year. We do not share your Confidential Information with third parties for their direct marketing purposes. We take reasonable measures to help protect your Confidential Information. However, no electronic data transmission or storage of information can be guaranteed to be 100% secure. Please note that we cannot ensure or warrant the security of any information you transmit to us, and you use the Site and provide us with your information at your own risk.
CHANGES:
We may update and post revisions to this Privacy Policy from time to time and will update the Effective Date when we do so.
